At Preferred Care Partner (PCP), our Daily Tasks/Shared Living service is tailored to support Participants in managing their daily living tasks while maintaining their independence at home and in the community.

Our Daily Tasks/Shared Living services encompass:

• Assistance with personal care: Our trained staff provide support with personal care tasks such as bathing, dressing, grooming, and toileting in a person-centered manner, ensuring Participants’ dignity and privacy are respected.
• Assistance with household tasks: We assist with household chores like cleaning, laundry, meal preparation, and grocery shopping, prioritizing Participants’ safety and comfort.
• Assistance with medication management: Our staff assist with medication reminders, administration, and monitoring Participants’ responses to medication, ensuring they have the necessary support to manage their conditions.
• Assistance with transportation: We provide support with transportation, including accompanying Participants to appointments, running errands, and helping them access community services, enabling them to maintain their independence.
• Assistance with shared living: Our shared living service allows Participants to live with a support worker, fostering independence in a safe and supportive environment.
• At PCP, we are committed to delivering high-quality NDIS supports tailored to the unique needs of our Participants. Our range of services includes Supported Independent Living (SIL), designed to promote independence through personalized support with daily tasks like personal care, cooking, cleaning, and budgeting.
• Additionally, we offer Short Term Accommodation (STA) to provide temporary housing and support during transitions to more permanent living arrangements. Our team assists Participants in finding suitable accommodation and settling in comfortably.
• For those awaiting long-term housing solutions, our Medium Term Accommodation (MTA) offers temporary support for up to 90 days. While MTA funding covers accommodation costs, it’s essential to note that it does not include day-to-day living expenses or personal care supports.
