At Preferred Care Partner (PCP), we offer a service called Assist-Life Stage Transition, aimed at supporting individuals with disabilities through important life changes. This service helps Participants with things like moving out of home, starting school or college, or getting a new job or home.

We understand that life changes can be tough, especially for individuals with disabilities. That’s why we’re dedicated to providing top-notch Assist-Life Stage Transition services that support Participants through these changes and help them achieve their goals. Our services are tailored to each Participant’s needs and provided in a way that focuses on the person.

Our Assist-Life Stage Transition services include:

  • Helping Participants with planning and preparation: We work with Participants to figure out their goals for the future and help them plan for the transition. This might involve identifying the skills and resources they need and supporting them in acquiring them.
  • Supporting Participants during the transition: We offer practical and emotional support to Participants as they go through the transition, helping them handle any changes or challenges they encounter. This could involve support with finding housing, jobs, education, or other aspects of their life.
  • Connecting Participants with relevant services and supports: We assist Participants in connecting with services and supports that can aid them during the transition. This might include connecting them with schools, employers, or disability-specific services like employment or accommodation services.
  • Helping Participants maintain their independence: We provide support to Participants to help them stay independent during the transition. This could involve assistance with personal care, household tasks, or anything else they might need help with.
  • Providing ongoing support and review: We continuously support and review Participants’ progress during the transition, making any necessary changes to their support plan as required.”
